Abstract Purpose: Urinary tract infection (UTI) from manipulation during percutaneous nephrolithotomy (PCNL) developed occasionally despite prophylactic antibiotics cover. There are various putative factors and varibles that may predict the development of post operative UTI and this study was aimed to bring to light the relationship between these variables and UTI in relation to PCNL. Patients & Methods: The medical records of 100 patients who underwent PCNL were reviewed. The data recorded and analysed for association with UTI were age, sex, stone size, pre-operative urinalysis, staghorn calculi, presence of hydronephrosis, operation time, residual stones. Post-operative Systemic Inflammatory Response Syndrome (SIRS) in the absence of other overt sources was used to define UTI. Results: We analysed 100 patients (57 male and 43 female) with a mean age of 49.6 (22-75)years. Mean stone size and operation time was 3.15cm (1-10) and 107.5 minutes (70-200), respectively. 17 patients developed SIRS, pre-operative pyuria was associated with SIRS (p=0.048). Other factors was not associated with post operative SIRS development. Conclusions: Only pre-operative pyuria has strong association with post operative SIRS. Consquently patients with pre-operative pyuria have to be treated with prospective antibiotics and post-operative intensive care.
